$200 — your home-visit service
Your service includes:
• A visit to your home
• Fitting the sleep-study equipment
• Delivery of the completed report to you and your referring doctor
Referral & screening
A GP referral is required. For the questionnaire-based pathway on our referral form, you need:
Epworth Sleepiness Scale: 8 or more
AND one of the following:
• STOP-Bang: 3 or more
• OSA50: 5 or more
CardioScan checks your referral and completed questionnaires to confirm eligibility for MBS item 12250. This is a referral and screening review — a separate specialist consultation with you is not required for this pathway/
Screening scores alone do not guarantee Medicare eligibility. If your scores are below these thresholds, your referring doctor can advise on the next steps; a different pathway involving specialist consultation may be needed.
Your study, step by step
1. Request your visit
Share your suburb and preferred callback details. We’ll explain how to submit your referral and questionnaires securely.
2. Referral check
Your referral and screening information are reviewed through CardioScan. We confirm the arrangements and answer your questions.
3. Setup at home
Once eligibility, location, appointment time and equipment availability are confirmed, we visit your home and fit the equipment. We’ll agree collection arrangements with you.
4. Receive your report
When the report is complete, we deliver it to you and your referring doctor.

Please keep medical reports, Medicare details and completed referral forms out of general website enquiries. We’ll explain how to provide your clinical information securely.
